A class I medical certificate is required for commercial pilots. The holder of this medical certificate shall be mentally and physically fit to exercise safely the privileges of the applicable license. Minimum age to apply for class 1 medical is 17 years. Class 1 medical is categorized into three types which are as follows:
(a) Initial Medical: Initial medical is done when a person with a valid Class 2 medical applies for the first time for a Class 1 Medical (a requirement for commercial license).
(b) Re-initial Medical: In case the last medical date (Class 1) is more than two years old, a fresh initial (Re-initial) Class 1 medical is to be conducted only at Air Force Medical Boarding centers, along with approved NOC by Medical Cell, DGCA.
(c) Renewal Medical: Class 1 Medical Assessments are valid for one year for persons less than 40 yrs. So a Pilot has to renew it every year.
The DGCA has made some changes to the medical application process, eliminating the need for the PMR forwarding system. This is now handled through the eGCA portal.
Here are steps to be followed for Class 1 medical exam (DGCA class 1 medical test list):
Step 1. Check whether the intended medical is an initial or a renewal medical.
Step 2. Appointment-
eGCA provides a short manual for the application process. Please read it thoroughly before proceeding with the application.
Step 3. No Objection Certificate (NOC); (if applicable)
For Re-initial /delayed/ early/ Post TU or Special Medicals only, kindly obtain a No Objection Certificate (NOC) from Medical Dte at DGCA, New Delhi. The NOC form is available in eGCA Website.
Step 4. The Investigation Reports-
You need to go through a series of pathological and radiology tests such as –

The above tests are for Class I Initial Medical. You need to carry these reports when you report at the IAF boarding centre. For Civil hospital, they will do the tests then and there itself.
Important: All tests must be done at NABL/NABH certified laboratories; otherwise, DGCA will not consider it a genuine Class 1 Medical. Some doctors may have their own clinic for the tests, but if not, make sure to get them from NABL/NABH certified laboratories before visiting the class 1 medical examiner/ IAF boarding centre. Please note that the medical reports must have a barcode / QR code for verification.
Step 5: Uploading reports on eGCA website-
The IAF Boarding centre personnel will guide you for re-validation of reports and necessary workflow. In case of civil doctors, they will provide assistance too. For more information, you can refer to the eGCA manual.
